Please fill out this form completely if you are interested in a performance slot at the 9th Annual Josie Music Awards Weekend of Events. There will be a preliminary and final selection.
If you are chosen in the preliminary round you may be asked for an online audition.
Please only complete this form if you are certain you will be available on any or all of these dates: JMA Fest October 20th, JMA GALA October 21st, Josie Music Awards October 22nd.

NOTE: All performances are determined by a selection committee and involve a submission and review process. Forms that do not have all of the needed information will be discarded.

Attending The 9th Annual Josie Music Awards on October 22, 2023 is deemed as consent in appearance in film, stream, or other media/materials video or still shots for promotional purposes.

Completing this form does not guarantee a performance slot.
I understand this is an unpaid performance. My band and I will receive complimentary entry to the event we are performing at . You will be sent a performance agreement with all the details. Your performance slot is not confirmed until this agreement is completed and received back in our office. Please note that this performance agreement has an independent music award show non-compete clause this year for 12 weeks prior to the Josie Music award show ceremony created by our industry leader panel due to the growth of our event/organization in the industry. The submission deadline for performance applications is June 15, 2023.
