NOTICE:
The 7th Annual Josie Music Awards on September 18, 2021 is being filmed for television, live stream, and other media purposes. Your presence at this event will be deemed as consent in appearance in the film, stream, or other media/materials. If you are selected as a performer you will receive a television release form to complete allowing rights to have your performance & song filmed for television, live stream, and other media.

Completing this form does not guarantee a performance slot. The Josie Music Awards will be held on Saturday, September 18, 2021. The Preparty is scheduled for the evening of September 17, 2021. SAVE THE DATES!
I understand this is an unpaid performance, myself and my band will receive complimentary All Access VIP Tickets to attend both days of events. Nominated artists performing may not use tracks or lip sync. You will be sent a performance agreement with all the details. Your performance slot is not confirmed until this agreement is completed and received back in our office. Please note this performance agreement has a non-compete clause this year for 8 weeks prior to the Josie Music award show ceremony created by our industry leader panel due to the growth of our event/organization in the industry. Submission deadline for performance applications is May 30, 2021.
